This is an interesting story, though I do have a few notes. First, why is it called Axis II? I assume this is in reference to the Axis alliance from World War Two but I could be wrong. Second note, you say where the anomaly is located in the Containment Protocols rather than the Description where something of that effect would be better located. The language in parts is clunky, but I fail to list any specific examples. Other than those, fairly minor, issues, Four Stars.

Personnel inside RPC-992-A must remain 100m away from RPC-992-B.

From my impression when you step through the mirror you transport to the other side of the mirror, already inside of RPC-992-B. There's no possibility to stay 100 meters away.

Any information regarding RPC-992-B's contents other than what is described in this document and the documents available to MSTs Delta-4 and Delta-5, is to be considered a Level-5 Security Breach.

This sentence should be reworded. Whatever you are describing here does not come across correctly.

RPC-992-A instances cannot be accessed by any other means.

There's some clunky wording in the containment and description. Read it all out loud to try and catch more of them.

This article is NOT based on Croquembouche and djkaktus' SCP-3790. However, as the format is very similar and they did it first, I'll suggest you read it.
I'm trying to do a wholly different thing on this article, and I hope that's noticeable. I'm open for discussion on this subject should a difficulty arise.
